New employees must enroll in benefits within 30 days from their date of hire in order to enroll in medical, prescription, vision, and dental coverage. Even if you do not wish to enroll, you still must complete the enrollment process in Bronco Hub and to waive coverage.
If an employee doesn’t enroll within their first 30 days, the next date to enroll would be during Open Enrollment, which is your annual chance to enroll or re-enroll in a flexible spending account or change your medical plan without a qualifying life event.
STEP 1: COMPLETE ENROLLMENT
Human Resources and Workforce Strategy is introducing a more streamlined, confidential experience through Bronco Hub, Boise State’s online system. Review the Benefits Elections Checklist and Guide to determine what elections you would like to make. After completing the checklist, follow step-by-step instructions in the Benefits Enrollment Job Aid to submit your elections through the enrollment process in Bronco Hub.
STEP 2: COMPLETE REQUIRED FORMS
- Basic Life Insurance Beneficiary Form
- Basic Life Insurance Minor Beneficiary Form (Only if choosing a minor as your beneficiary)
STEP 3: COMPLETE OPTIONAL FORMS
- Classified to Professional Retirement Election Form
- Supplemental Retirement Account (SRA) Authorization Form
- Voluntary Life Insurance Enrollment and Beneficiary Form
STEP 4: SUBMIT BENEFIT FORMS
Once you’ve completed your online benefits enrollment in Bronco Hub, you can submit the remaining required and optional forms using any of the following methods.
Option 1: Email
Print, sign, and scan your forms or use Adobe Acrobat Pro to sign and download them, then email the completed forms to benefits@boisestate.edu. Please do not include your or your dependent’s Social Security Number (SSN) for privacy purposes. Our Benefits Team will call you to get that sensitive information, or you can call Benefits at (208) 426-4450.
Option 2: Secure Share
For secure document sharing, upload your benefit forms to Virtu. Sign in with your myboisestate username and password, and easily upload your documents. The platform is secure, allowing you to safely include Social Security Numbers on your forms.
Option 3: In-Person
Print, sign, and drop off your benefit forms at Human Resources, or visit the office for assistance from our Benefits Team.
