DBS Vehicle Request Form

MOTOR VEHICLE CHECK: Required if the employee will be working in a position that A) is responsible for operating a university-owned or controlled vehicle for any purpose, or B) is responsible for transporting students, clients or others in a private vehicle on a routine basis, or C) is responsible for transporting minors under the age of 16 at any time. Optional (but recommended) for volunteers or student employees who will work in a capacity that meets any of the above conditions.
